SUMMARY OF ARTICLE

Preserving the Option of Vacuum Extraction; 5 Experts Tell Why and How

This article, published in the February issue of OBG Management is a question-answer format between five experts in vacuum extraction.  Neal M. Lonky, MD, MPH (facilitator of discussion), James Bofill, MD, Thomas Garite,    MD, Robert Hayashi, MD, and Victor L. Vines, MD.

The article covers the decline of operative vaginal deliveries due to the fear of litigation.  The experts  explain why vacuum deliveries should continue and how to reduce complications through sound patient selection and proper technique, which reduces medical legal risk.  The experts agreed that pre-delivery checklists, competency assessments, and reporting forms would help improve operative deliveries.  A sample delivery report form is included with the article.

Vacuum-Assisted delivery: Improving Patient Outcomes and Protecting Yourself Against Litigation
There are three articles in this supplement to the February issue of OBG Management.

Vacuum-Assisted delivery, Practical Techniques to Improve Patient Outcomes.    By Aldo Vacca, MD
In this article, Dr. Vacca points out that “Attention to the details of technique can help prevent problems and ensure the best patient outcomes.”  Dr. Vacca teaches the flexion point, performing the procedure, infant care after vacuum assisted delivery (VAD), and physician training for VAD.

The Physics of Vacuum Extraction; Proper Use of Compression and Traction for Better Patient Outcomes.    By R. Gail Billings, PhD
This short article discusses the mechanisms of VAD, how much traction should be applied, in what direction, and it’s effects on the fetal scalp.

Establishing a Protocol; The first step for reducing potential for litigation.  By Victor L. Vines, MD
The article addresses the possibility that VAD, and forceps deliveries are trending towards extinction because of the risk of litigation.  The article discusses the statistics of how often VAD’s are performed; the reasons litigation takes place, and steps to take to prevent problems.  Dr. Vines instructs; selecting the most appropriate mode of delivery, avoiding litigation claims in your practice, and recommendations to promote safe use of VAD.

A Comprehensive Workshop for Residents and Fellows
Clinical Innovations is sponsoring VAD workshops conducted by our speaker’s bureau of Aldo Vacca M.D. and Ross W. McQuivey M.D.  This workshop reviews techniques to reduce the risk of vacuum assisted deliveries and gives physicians the background knowledge and skills to safely perform vacuum-assisted vaginal deliveries.  A full spectrum of vacuum assisted deliveries will be presented and critically reviewed using an interactive lecture, case-study, and hands-on format.

With the training and material provided, this workshop establishes a foundation for the residency program to repeat the educational efforts for years to come.  Please contact Clinical Innovations to schedule a workshop.
